Registration for Radiation Work at SPring-8

For domestic users:
1. Every experiment participant must complete and sign 2 copies of the form and mail them to User Administration Office 2 weeks before the experiment starts
2. Every applicant (NSRRC employees, not including NSC partime employees, do not need to submit health examination report) should submit health examination report (within 1 year) to NSRRC User Administration Office
3. In order for SPring-8 to accept form 5-1, please make sure that your SPring-8 user information include NSRRC as your secondary affiliation. If it is not listed, please add
NSRRC to the affiliation list
For international users (outside of Taiwan)
Please complete the form, have your affiliation certify the form, and send the original copy to SPring-8.

Applicant should print out the online “SPring-8 Users travel reports” form,
put your signature in the applicant’s signature column, and have your P.I. sign the form. Later,
please send the form along with the receipts/invoices and electronic experiment report file (1 report for each group) to User Administration Office. |
